Person Initiated Follow Up (PIFU)

You will have direct access to the service for one year, from the date of your last appointment.

If you have concerns or worries about the problem you spoke to us about, contact us. A clinician will review the details and a member of the team will be in touch.

We can telephone you or arrange to see you in one of our clinics.

If your open return has expired, please visit our website to access our services, or alternatively speak to your First Contact Practitioner (FCP) or GP at your local practice.

A short questionnaire will be texted or emailed to you. You need to complete this so we can understand what you need.

Alternatively you can call our team who can assist you in completing your PIFU questionnaire.

Depending on what you need, our team will then be in touch to help. Further information about our wait times can be found on our website.

Follow Up (Booked)

In your appointment it was felt a further appointment with your clinician would be helpful for you.

An appointment will have been booked for you to see your clinician again.

If you do not have a record of this please call us.

Discharge

From our appointment today you and your clinician felt that you would not need further care from our service.

If your condition changes and you would like to be seen again you can call us anytime in the next 6 months and rebook with your clinician.

It may also be that you need to be seen by a different service. If this is the case we will have informed your GP.

Referral to MSK Class

In your appointment today it was agreed that our MSK Classes may be beneficial for you. These classes allow you to perform your exercises while being supervised by our clinicians. They are on hand to support you and help you perform the exercises at a level that suits you.

You will have been added to the waiting list for one of these classes. The classes team will be in touch to arrange this once you reach the top of the list.

Referred to AP

Following your appointment it was felt that you needed to see a member of our Advanced Practitioner team. APs are more specialised and able to define treatment for more complex conditions, or those presenting with rare symptoms.

You will have been added to their waiting list and will be contacted once an appointment becomes available.

Diagnostic Referral

Having seen one of our Advanced Practitioners it was felt a diagnostic scan was needed. This scan will be used to confirm or rule out suspected diagnoses.

These scans are performed by other services. We will send the referral to them and they will be in touch to arrange an appointment (there is usually a wait list also).

Once we have recieved the results from the scan (this can take 6+ weeks) we will be in touch to arrange an appointment.
